Incident Reporting System

  • If there are ever any incidents within the healthcare sector, they need to be recorded in an Incident Reporting system.

  • The application needed improvement, users were inputting incorrect information, due to lack of knowledge of site usage; this affected data integrity and analytics.

  • Colour and typography needed to be kept the same as the existing application.

  • Please note that for GDPR reasons, all information provided is fictional.

  • Below shows the step by step prototypes in the journey. The user will need to click ‘Add Position’ to start the incident report.

  • For error handling reasons, the full application does not display until the beginning section is filled in.

  • Information input into the system, shows as a tick in the left side bar, the user can then proceed to the next part of the application. By doing this, the user cannot miss any information from the report one area at a time.

  • Dropdowns were created for the user to input; these contained default answers and allowed the user to fill in the sequence of events over a time period. By doing this, the user can make note of each patient movement and the reasons for their movement change.

  • Once the user fills out all information for each section and they are marked as complete, a plus sign is generated on the next section for the user to fill in.

  • With this system, user will be able to input body parts used in restraint, a staff member involved and whether there was a staff swap.

  • When the section is complete, a tick and edit button appears for the user to make amendments; when accurately completed, the green tick confirms.

  • A summary of information appears on screen to allow the user to amend or complete the section.
